Requests for Medical Reports and Records
To request medical information, kindly ensure that you have:
- The Patient’s written consent is required for the release of medical information;
- For patients below 21 years old, the consent of the patient’s parent or legal guardian; or
- If the patient is deceased, a copy of the patient’s death certificate and written consent of the patient’s estate must be produced.
Patients can request for medical reports while one is still being warded and before discharge.

Fees for Processing Medical Reports
$150
Additional charges such as consultation, X-ray, laboratory tests, or any other charges that may be incurred in the preparation of the medical report are not included in the fee for processing the medical report. These will be charged separately.
An administrative charge of one-third of the charge of the medical report is applicable if cancellation/withdrawal of request is made when the medical report is already being processed.
Please post to us the completed form together with the cheques for such fees can be made payable to “Soo Khee Chee Surgery”. Payment by cash, NETS or credit card is available for requests made at our clinic. We will begin preparation of the report once your payment has been received.
Once ready, we will have the report couriered to the chosen address indicated on the form. We will contact you with the tracking number once posted. Alternative, if you wish to collect the report yourself, we will contact you once the documents are ready for collection.
